What Causes Muscle Pain In Lower Limbs And Fatigue?

Question: Hello doctor



I was taking amvasac 10 mg in morning.

approvel 150 mg in the night.

In addition, I was taking 10 mg Lipitor. (450 mg wellbutrim, 10 mg cipralex innight)



My pressure was 90 / 130 or 90 / 140

All was well.



then I had muscle pain in lower limbs.



CPK was elevated to 260.



So my internal medicine doctor did the following changes.

1. He stopped amvasac and approvel in night

2. He stopped lipitor



He asked me to take

co approvel (150, 12.5 in morning)

instead of lipitor, he put me with zocor.





my BP climbs to 100 / 140.



I feel tired and am not able to convince my doctor that my BP is not controlled.



My weight was 92 Kg with 168 cm.



I switched to a low calorie / fat diet.



Please advise me

You should stop Tab zocor.

Detailed Answer:
Hello. Welcome to HCM.
I have read your question and understand your concerns.

Tab zocor contains simvastatin, it's of a same group of drugs you are previously taking. Two most common side effects you should remember of this medicines -
1. Muscle weakness and injury
2.liver injury.

The CPK levels are already elevated, however CPK - MM is a more specific marker for muscle injury. Your doctor will know about this.

Your doctor has continued same group of medications in spite of you have already muscle injury. Further your SGOT (liver injury marker) is also marginally elevated. This all effects will be decreased once you stop Tab zocor.
There are other less toxic medications available in market. Ask your doctor about tab ezetemibe, finofibrates etc. I can't prescribe you here. All your weakness and body pain is due to that only not because of blood pressure.

Now regarding your blood pressure, it's still under control at present, however diastolic blood pressure is still high. Target should be less than 140 /90 mmhg. I defer in that treatment also. Your old combination of BP medicines are better than newer drugs. So ask your cardiologist to again restart same medications.

All other reports are normal.
Do daily brisk walking, cycling, yoga and dietary restrictions.
