default
Posted on Tue, 23 Dec 2014
Question: Hello doctor

I was taking amvasac 10 mg in morning.
approvel 150 mg in the night.
In addition, I was taking 10 mg Lipitor. (450 mg wellbutrim, 10 mg cipralex innight)

My pressure was 90 / 130 or 90 / 140
All was well.

then I had muscle pain in lower limbs.

CPK was elevated to 260.

So my internal medicine doctor did the following changes.
1. He stopped amvasac and approvel in night
2. He stopped lipitor

He asked me to take
co approvel (150, 12.5 in morning)
instead of lipitor, he put me with zocor.

my BP climbs to 100 / 140.

I feel tired and am not able to convince my doctor that my BP is not controlled.

My weight was 92 Kg with 168 cm.

I switched to a low calorie / fat diet.

Please advise me

Brief Answer:
Should discuss with your cardiologist the use of Statins...

Detailed Answer:
Hi XXXX

I have gone through your medical history carefully and understood your concerns.

You are reporting to have muscle pain in the lower limbs while taking Lipitor. Your doctor has stopped Lipitor and put on Zocor. Both of them are belonging to statin drug group. One of the side effects of them includes muscle pain and increase in CPK.

I have gone through your reports, and noted that CPK was slightly elevated. To my opinion, you can discuss with your doctor to stop taking statin of either name (Zocor or Lipitor) as they will have the same side effect on muscle.
However, I need to see your lipid profile result first to finally determine if you really need statins.

Hope it was of help!

Brief Answer:
No need for statins, continue with diet&lifestyle changes...

Detailed Answer:
Hi back XXXX

Thank you for providing your lipid profile reports.

I have gone through the numbers there, and I have noticed that they are ok, except for triglycerides which are slightly elevated and indicative that your diet is rich in foods high in carbohydrates and fats.

To my opinion, taking into account your overall health picture and lipid profile, I advise to stop Zocor and any other statin drug. For the moment, you should focus on implementing good diet and lifestyle regimen:
- no fatty/sugary/processed foods
- more fruits and vegetables
- less meat, more fish
- increase physical activity (walking/jogging/running).

Hope it helped!

Thanks doctor

I have done severe changes in food. I having a balanced low calorie and no fat diet.

But irrespective of that what is the chances of LDL and Tri shooting up.
If it does, how we can find that before it happens.

I am on statins for 10 years and never had this issue.

I suspect amvasac (norvasac)

What is the smallest possible time it can go up ?

Thanks

Brief Answer:
Norvasc rarely associated with muscle pain problems...

Detailed Answer:
Hi back XXXX

Norvasc is rarely associated with leg pain. Swelling of ankles and legs is commonly occurring while using this drug.

I still think statins are the culprit for your pain. You can stop using any type of statins during these days to see if any changes.

If still the same pain, then, continue with co-aprovel only. If the pain persists, blood electrolyte level is required to rule out electrolyte imbalance.

Apart diet changes, medications you take can also influence the lipid profile as they will interfere with hepatic metabolism.

Hope it was of help!

Brief Answer:
Diet&medications you take are possible causes...

Detailed Answer:
Hi back XXXX

In the earlier discussion, I've talked about the role of the drugs you are taking in the liver. All of them will be metabolized by the liver. Thus, liver will be work-overloaded, which is also reflected in your lipid profile.

Furthermore, you are also using the diuretic named hydrochlorothiazide, the use of which is associated with increased cholesterol numbers.

High triglycerides level is reflecting:
- what you eat
- medications you take

Hope it answered to your queries!
