Brief Answer:
he has to go to the ER

Detailed Answer:
Hello,

breathing difficulties have to be investigated always! His medical history makes heart failure the most likely scenario. Fluid collection in the lungs is possible. Other equally (or more) serious possibilities may apply, like pulmonary embolism. A COPD exacerbation is also possible.

He has to be examined by a doctor to determine the cause of his dyspnea. In many cases the cause can be treated successfully. For heart failure, diuretics are the mainstay of treatment.
