What Causes Numbness In The Penis Post A Surgery?

Question: I had slight Testicle torsion (in right testicle) back in December (the left testicle was removed when I was young). I had the surgery and it was treated well. unfortunately, after the operation with 5 months or more, I still get pains in the testicle.so the urologist had an ultrasound test and it was fine.so the urologist wanted to make sure it is an infection or some pain. He asked me for sperm analysis test to check for infection and result was no infection, then the doctor decided to go further with hormone test and the hormone test was testrone was low testo 251 ng/dl. he advised me with hormone therapy but I refused.

The question now, I still get pains when I carry something or do some sport or even any big move, when I seat on a high chair or long on the desk, I get the whole area numbness including my penis and the testicle. as soon I stand and walk a bit I feel the rush of blood in the whole area. when I try to touch the testicle, it feels like pain in the side either it was the same place the operation or nerves, but I can't see anywhere stitches or I don't urinate blood. last advice I got from the urology, as more I get pains, I take some pain killers. can you advice what to do or what kind of thing happening to me here? especially the pain and numbness are not constant, sometimes are going high and sometimes a day or two passes without pain. concerning my sexual life, I still get an erection and am able to do sexual intercourse normally.

Hi

I don't know what type of surgery was done for torsion but usually we do testicle fixation and detorsion. Fixation means than testicles is stitched to inner fascia with sutures and in this case small testicular nerve may be entrapped between sutures and cause chronic pain that you are describing.
Second, there may be stenosis of blood vessels that supply testicles. This is also complication of surgery. third, there may be chronic epididimitis or orchitis.

In every case, you need to take painkillers such as Diclofenac or Ketolac when pain occur, avoid tight innerwears and avoid longer stay in too cold or too warm environment. Also, smoking should be avoided.

Some more sophisticated tests to find exact disorder include scintigraphy and angiography of testicles. it will reveal possible circulation issues. Entrapped nerve can be cured only by another surgery and this is the last option, At this point, if there is no significant impact on your sexual life then I don't think that surgery is required.

Dear Dr Rommstein,

Thank you for your feedback, I did the surgery in Germany, so am not sure which way was it done. my question now, if I decide to live with pain and adapt with it, will it affect on long term on the testicle size and may lead to losing it (such as currently i had testo 251)?

Well, only if there is significantly reduced blood flow to testicles, then atrophy may happen, but if there is no decrease in size over these 6 months and good testosteron levels then there is no reason to be concerned about such disorders in future. If pain progresses, then you should do Doppler or scintigraphy to see the blood flow inside testicles and then we can react.
