What Causes Pain And Burning Sensation In Right Testicle?

Question : Hi, I am 33 years old and married, I am having pain and burning discomfort primarily in my right testicle. Following are the symptoms:

Pain comes if I sleep on my stomach and there is pressure on my testicle.
Pain comes if I drive my car, applying clutch often due to traffic. This also gets pain in my left testicle. After long drive I can even feel pain in my testicle while walking.
Pain increases when I am hungry like an hour before lunch or dinner. It gets better after taking food.
Any activity where I stretch my legs such that testicle are also stretched gets pain.
Mild pain and burning feeling comes after ejaculation and sometimes few hours later.
Pain sometimes radiates to right leg. Sometimes it is a pulsating pain.

I have this problem for almost 1 year, but now it has increased.
Visually there is no swelling or enlarged veins. But the center line in the scrotum separating the 2 testicles seems to have displaced a bit. The problem had started during masturbation as a dull ache and has gradually increased.

I went through ultrasound of scrotum (twice), semen culture & analysis but there were no abnormalities. Sperm count was normal as per the Urologist I met.

I have reviewed your reports and of the opinion that excessive strain in addition to mild varicocele is cause of pain.

Since it is mild surgery is not indicated unless otherwise there is severe pain disabling your activity.

In my opinion slight change in personal life with use of scrotum support will suffice to deal with pain.

If you have not started it already start using as soon as possible.

Thanks for the reply.
I have used scrotal support in the past, it did help but the problem was not going away, besides it was very uncomfortable.
You have mentioned excessive strain, but I am not doing any extra stressful activity like playing sport etc. I have almost stopped car driving. Is this a muscular/nervine weakness in testicles? Is there a way to improve this condition with some oil massage or some other medicine?

The excessive strain add on pre existing varicocele causes the problem rather than single problem.

I appreciate your effort to avoid strain but the conditions which impair drainage of blood (Sitting in small place, same position over a large time, Using tight underwear, excessive stretching of groin etc) need to be avoided so there is minimal discomfort.

The pain occurs due to venous congestion and not due to venous or muscular weakness.

The doctor I met earlier said there is no varicocele. Less than 2mm should not produce any symptom.
He mentioned that it could be orchitis due to non specific infection or due to psychogenic origin. He recommended me to take Ventab ( desvenlafaxine) 50 mg a day for 3 weeks to treat a condition called Fibromyalgia.
What is your recommendation?
Also, As far as I know the only cure for varicocele is surgery. Is there any medication which might improve the condition?

The persistent problem with well defined triggers and localized pain to testicle is not indicative of fibromyalgia. I request you to read something about fibromyalgia for your awareness.

He is right in saying that generally 2cm varicocele do not cause symptoms but it does not mean that it will not cause any symptoms in 100 % of the patients.

Exceptions are rule in medicine and it is possible that even subclinical varicocele can cause symptoms.

There are no medications for varicocele.
If it is mild scrotal support is recommended.
If severe than as you know surgery is option.

Could it be chronic orchitis or any other infection? even Filaria? I have mild anal fissure for almost 2years. I just want to rule out possibilities. Are there any tests that can rule out these infections?

I do not think there is any infectious pathology as evidenced by clear history, no response to antibiotics and response to scrotal support.

The are numerous tests for each and every possibility but there is no point in going for them as the possibility of infections negligible.
