Question : sir , i am male , 28 years , no medical condition , i have been doing cycling regularly for 3 months for 15 km daily , last week i felt mild pain in my left testis , then when i masturbated the pain aggravated , while ejaculating i felt little obstruction and pain , after ejaculation also i felt pain....i used to masturbate 2-3 times a week but for longer time(usually 1 hour each session) , after 2-3 days intensity of pain reduced , but it is enough to cause discomfort ..it like sore in my left testis... i consult urologist ..he asked me for scrotum ultrasound which came out normal with mild left epididymis prominence compared to right one .........others things are normal...no variocele, hydrocele , no inflammation in testis etc etc. he was confused ... he told me that either my muscle is strained ....or mild infection in testis ... but there is no fever ...no scrotum inflammation....no pain in urination ...no blood in urine ...so he told me to wear supporter and advice me some anti inflammatory medicine ..as he wasnt sure i didnt take medicine........also now from two weeks i have this pain which comes and go............no pain in morning...pain at night and ......no pain when i am resting whole day...but any physical activity i do ...it cause me pain at night in testis ............very low sex libido.........on erection it pain.........hardly able to maintain erection for even small amount of time .

From the available description it is clear that the pain is muscular rather than anything testicular or serious.
This is evidenced by relieving of pain on rest, no other obvious sign of disease and almost normal ultrasound report.

Firstly in muscular pain the treatment of choice is scrotal support and application of ice packs on local area. For cold application you can keep ice packs or cold water packs in trouser pockets which will help you to relieve pain.

Secondly you need to understand that scrotal support should be maintained for whole of day when you assume erect position. Also there is need to use loose clothes during night/sleeping hours.

Thirdly there is no need to take painkiller and it will be alright within a one to two week period with above measures.

In regard to masturbation try to avoid for two weeks before pain is completely resolved.

sir , today i had my cbc and complete urine test....urine test is normal........but cbc test results are

wbc 9.3
lymh 4.5 and its % is 46%
mid .5 and 5.1%
gran 4.3 and 46%


hgb 16.1
rbc 5.71
mchc 30

rest is normal......also sir i can feel pulse vibration in my left testis.....nerve connected from testis to penis .....palpations are there in that nerve....also i want to know how much period i have to wear supporter to see the results...nd why i am felling low sex libido and low erections

Regarding duration of scrotal support you need at least one to two weeks to see good results. In addition try to use it most of time when standing position.

The nerve you are mentioning is called as epidydimis a structure which contributes the seminal fluid and stores semen. This is the structure which is most affected with sternous activity.

In regard to decreased libido and sex drive the cause is stress associated with pain. As we know sex is pleasurable activity but it needs stress free mind to have proper activity.
In my view wait for relief of pain and I hope with that you will experience full fledged sexual activity.

respected sir,
thanks for replying
i have provided u with my blood report.....do u feel any type of infection , as it stated that lymphocytes level as well as % is higher than normal......what are the other sign i should look for ...to strike out any other problems?
my urine sample is normal...could this pain can be related to kidney stone ?.....i had 5mm on left side kidney detected 4 weeks earlier...but i feel no pain ....in left abdomen(both upper and lower)
doctor also told me to apply warm water with salt in it on scrotum area to relax my muscle....is it advisable by you ?

First of all I like to tell you that lab reports are interpreted in light of clinical symptoms. 45 % of lymphocytes are slightly up than normal but it can be normal or lab error since the clinical symptoms do not support any such elevation. In addition lymphocytes rise in chronic conditions and not in short term conditions as in your case.

There is no symptoms which supports renal stone. There seems to be no link in two conditions.


Yes application of warm water is advisable. Use of scrotal support keeps the area warm and helps muscles to get relief. If you apply warm water in addition to support it is likely to work better. At least it will not harm you in any aspect.


You need to take the recommended things rather than searching for big disease. Al the reports are supporting that nothing to grievous and earlier acceptance of fact is desirable.
