What Causes Pain And Discomfort In The Left Abdomen Post An Ileostomy?

Question: I had an ileostomy in February. I have a dull ache and discomfort in my lower left abdomen that radiates to my back. I am feeling nauseous also. I am 68 years old.

I am Dr Sunil a Gastroenterologist and would be answering your query.

According to your narration, that may be due to Pouchitis. It happens when an internal pouch becomes inflamed this inflammation leads to dull ache and discomfort in and around the area of stoma.As the dermatomes supplying abdomen are often overlapping pain in a particular inflammaed area can also be felt in other areas like back. It's a common complication in people with an ileo-anal pouch or continent ileostomy.
Second possibility is nephrolithiasis(kidney stone) or a possibile urinary tract infection.Pain is often felt in flanks are i sunilateral and radiates toward back

Non specific neuralgic pain related to irritation from ligament stretch / sprain or others is also a posssibility post surgery.

Initially i would suggest you to go for-

Ultrasound abdomen and a urine and stool routine test.

Kindly answer the following questions for a better approach-
Can you use a single finger to indicates the area or it is more diffuse.
The pain started from how long(after surgery)?
whether onset was sudden or gradual?
whether pain is continuous or intermittent?
Please describe the nature of the pain - stabbing, burning, gripping, ?
Is there are aggravating or relieving factors?
whether there is any radiation?
Your stoma care?.
Have you tried any antiinflammatory antispasmodic?
