In view of these, I would consider 2 possibilities.
The first and the more drastic one needing urgent care would be a possible heart condition. The heart issues in diabetics dont present with the usual chest pain. They present with your symptoms- shoulder pain with tingling and numbness. SO you should be advised to meet a cardiologist.
The second, a less serious condition may be the possibility of a radiculopathy. Simply put, nerve compression. So you need to consider getting a MRI of the spines, and if needed meet up with a spinal surgeon.
