Brief Answer:
Investigations, clinical evaluation and treatment

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Male/68 - swelling and pain in testicles - very red - taking painkillers - wants to know what it is...

Red, swollen and painful testicles are usually due to inflammation of the testes and epididymis and is called Epididymo-orchitis.

I would advise the following in such a situation:
- Send the sample of urine for routine, microscopy, culture and sensitivity before the start of the antibiotic to get a proper report that helps in near future for choice of antibiotics.
- Scrotal support.
- Plenty of oral fluids.
- Ultrasonography and color doppler. This helps to get a proper diagnosis whether inflammation or torsion or so.
- Tests of blood
- Consult your Doctor to get these tests done and to get started on proper antibiotic once the sample of urine and blood are given.
- Get a reference for an Urologist for further clinical evaluation, examination and proper curative treatment.
- Ultrasonography of the abdomen particularly for genito-urinary system.

I hope this answer helps you to get a proper diagnosis and treatment.
