Question : Dear sir
two days back I have done TMT 2 echo and ech. Everything is normal.
but sometimes my left hand side and neck side pinching and tightness. Then I feel anxiety. Yesterday I went to cardiology doctor and I ask him . All tests are normal and is there any chance to get heart attacks with in one month. He said yes
but reports all are normal including lipidpprofile.

TMT 2D ECHO I AM DOING EVERY 10 DAYS.

Dear Sir / Madam
1. It would have been great if you had provided your complete clinical profile. Anyway, the following medical facts will clarify your queries.
2. Pinching, tightness involving neck and hand which do not come up on exercise / exertion, which occurs spontaneously but is never precipitated by exercise, which lasts for less than a minute or for many hours together and is not accompanied by any other symptom is not likely to be related to anginal heart disease. Moreover, in case of normal TMT test with non-provocation of symptoms during TMT, the symptoms are unlikely to be anginal.
3. Despite all these test being normal, still you can have have heart attack in near future. Yes, this is true. Because these tests (stress test/ TMT) tell us that if there is any significant blockade in your coronary arteries or not (>70% and flow limiting). It also tells us that current symptoms are due to any coronary artery blockade or not. However it does not guarantee against any future heart attack in any manner because heart attacks occur mostly (85%) on those cholesterol plaques which are causing < 70% blockade and are non-flow limiting; hence can not be picked up by any stress test.
4. Even if we pick these plaques on angiography, it is of no practical use. It does not change the management substantially which in any case relies on preventive measures only. These include psychological stress avoidance, cessation of smoking, control of cholesterol/ diabetes/ high blood pressure and weight management & daily exercise.

There is no need to get these tests done every 10 days.
You should consult a physician who will examine you and guide you further that if there is a need to see a neurophysician or not. As I detailed you earlier, please do not worry too much and as per available information, these symptoms appear to be more from nerve related issue rather than heart.
