Brief Answer:
As detailed below.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/72 sudden onset - pain in abdomen around the belly button - with nausea - for the last 24 hours - recent colonoscopy negative - Sciatica is also hurting - Rectum also painful - wants to know whether to go to ER ...
Sine there are multiple problems like pain in the umbilical are, sciatica, pain in rectum and already 24 hours have elapsed, it is better to go to ER for the following reasons:
Clinical evaluation related to each symptom and examination and investigations as per the clinical findings and reports of the preliminary reports like that if blood, urine, stool, X-ray of the abdomen in standing position, Ultrasound etc.
Further tests like MRI of the lower spine to review the extent of sciatica any relation to the pain in abdomen due to neurological involvement or an actual problem if the small intestines or lymph nodes or diverticulitis and so on.
Also needed will be per-rectal examination and ano-proctoscopy to see whether there is proctitis.
I hope this answer helps to get an early and proper diagnosis which is the mainstay of the proper treatment.

Take care.
With Regards.
