Question : im having some pain in my testicle, on the epididimis head. some pain in the glans at the meatus. i had a hypospadia when i was young

Pain in Epididymis is mostly due to infection of Epididymis (Epididymitis).

Consult qualified Urologist for clinical examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound Scanning of Scrotum .

You need to take broad spectrum antibiotic like Cefixime along with anti inflammatory drug like Diclofenac twice daily for 2 weeks .

Ensure to drink ore water to keep your urine dilute

The infection of Epididymis and meatus could be possible if the surgery for Hypospadias is done very recently .

However if the surgery has been done long back it has no relation to this recent episode of infection .
