Brief Answer:
Mostly bladder infection

Detailed Answer:
Hi XXXXXXX,
Thanks for writing in to us.

I have read through your query in detail.
Tip of penis pain can be due to many causes.

1. Bladder infection is the commonest cause where the pain is referred to the tip of penis (distal urethra).
3. Sometimes stones in the urinary tract can lead to pain referred to tip of penis.
3. Prostitis or infection of the prostate gland can also be the reason for such a pain.
4. Tip of the penis skin is very sensitive and application of irritant soap can cause tip of penis pain.
5. STDs also cause such pain but you have got yourself tested negative.
6. Hygiene is important to reduce the occurrence of some forms of pain on the tip of the penis. This involves cleaning the penis daily and also taking a shower.

I suggest you consider the above mentioned causes leading to tip of penis pain and consult the urologist. If necessary a ultrasound scan on full bladder and urine culture test might be done.

Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek

Bladderinfection or prostitis

Thanks for writing back with an update.

Semen in urine during stool is a possibility in bladder infection cystitis and prostitis. Urine culture must be done when that happens. I wish you get evaluated by your urologist in detail when it happens again.
