Brief Answer: Surgical repair is the treatment of choice Detailed Answer: Hi there, Thanks for your query. Tendons, ligaments and the joint capsule help in maintaining the integrity of the shoulder joint, so that the ball (knob-like upper end) of the upper arm bone- which moves freely in the socket (a cup-like structure), without coming out (dislocate). However, in subluxation of the joint, the range of movement of the ball increases and, in a certain position (such as, when your wife raises her arm), it impinges on the overlying nerves of the arm. Irritation or pressure on the nerves causes severe pain in the region it represents. This nerve-pain is called Neuralgic Pain (Neuralgia). The only ideal and permanent treatment is surgical repair of the subluxated joint, before the nerve(s)supplying the entire arm are irreversibly damaged leading to arm's paralysis. Pain can be curbed to a significant extent by Nerve-Pain-killers (Neuralgesics), such as, some anti-epileptic drugs, (carbamazepine, gabapentin, Pregabalin). However, these drugs have their own side-effects, apart from their inability to give complete relief from pain. Consult your doctor and apprise him of my opinion. I am certain that he will agree with me and prescribe suitable treatment as advised, in appropriate doses. Hope I have addressed your concerns to your complete satisfaction and you find my response helpful and informative. I will be happy to answer further queries, if any. Fond regards and have a nice day, Col (Dr.) Rakesh Karanwal
