What Causes Pain During Bowel Movement?

Question: I have had these symptoms for over a year on and off. i have pain sometimes during bowel movement and then a streak of blood on one side of stool and then light blood on toilet paper that stops. I also have yellowish leakage that comes and goes but has seemed to continue for about a month now. It has a fishy smell? When it leaks and i do not clean it soon I get a horrible rash around my anus and crack. even when it is not leaking I have the fishy smell when i wash it. I assume it is a hemorrhoid or anal fissure?

Pain during bowel movement indicates that you may have anal fissure indeed. Fishy smell and yellowish leakage can be a sign of some abscess due to anal fistula. A year is a long time and you should not wait with it any longer. Please note that this symptoms can also indicate Ulcerative Colitis. This disease, if left untreated can lead to serious complications.
That is why you definitely should consult proctologist. Rectal examination should be done at first. Colonoscopy and transrectal ultrasound should be considered.
Anal fissure can be healed with medicines only if it lasts no longer than 6 weeks. After that time, surgery is the only way to fix it.

I do have a doctors appointment next week but when I told him about the issues last time he just told me it was probably a hemorrhoid and eat more fiber and vitamins. But looking up anal fistula i have no pain or lesions anywhere on the area just leakage from inside and the random blood on stool. Could it be IBS? the leakage doesn't always smell but is always a light yellowish color

IBS is a group of conditions. It is not a single disease. We put under the IBS every issue which is unknown for present medicine. Simply when some symptom of gastrointestinal tract can not be explained by any know disease, we call it IBS.
But before such diagnose, all known issues have to be ruled out. So at this point you can not say or even suspect IBS. Moreover, IBS does not give rectal pain.
Hemorrhoid can be the case, but this also needs to be diagnosed. Medicines like suppositories or sitz baths wont cure hemorrhoids either. Once the hemorrhoid appears, it stays there for life. So with this disease surgery is also the only possibility. Medicines can only make symptoms less present.
So your doctor is wrong. You have right to feel comfortable.And you should have it checked and property diagnosed. So please consult proctologist.
Leakage can be also a mucus. Characteristic for many issues including constipation, fissure, hemorrhoids, large intestine polyps, Ulcerative Colitis.

what can i do before my appointment next week to help with the rash and leakage issue? I had to leave work today as it was leaking quite a bit. Is there a cream or something I can use to sooth the area until next week?
