Brief Answer:
Pelvic inflammatory disease likely

Detailed Answer:
Hi dear,
I have gone through your question and understand your concerns.
Pain during intercourse, pain in lower abdomen and pressure on the anus is likely due to pelvic inflammatory disease. Other possibility is endometriosis, which can be ruled out by MRI.
I will suggest her to consult an OBGYN specialist to confirm the diagnosis and get appropriate antibiotics.
Hope you found the answer helpful.

Hello Dr Dippi

XXXX wrote on my behalf last week, concerning my health complaints.I would like to add and expand to that. Firstly, to date I have not had any major heLth issues , at 60 my weight remained constant at around 104lbs, balanced diet , exercise gets. Historically however, I have suffered from constipation, due to a difficult birth i developed piles. T!is condition has been under control, but two weeks ago I had an episode of severepain in the abdominal area and pressure on the anus , accumulated to a massive pain. Passing urine was painful too. This has now cleared, some discomfort when urinaxing but no major pain. Now, the doctor insists on a colonoscopy and if money was no object that would be OK, insurance does not cover the procedure.
My question is would there be a better way to screen both issues., Possible XXXXXXX issue or something wrong with my Bowes.
Sincerely
XXXX
