Brief Answer:
My suggestions

Detailed Answer:
Hi
I am Dr Mittal.
I have read your query.
I think I would be able to help you.
I feel that the painful condition means that the infection may have spread.
Now on antibiotics, if the infection has spread, it may suggest drug resistance.
So overall you may need a change of antibiotics, e.g. Azithromycin may be added.
As for the pain, pain killers may help you get sufficient relief. Sometimes, I add a steroidal cover. But in presence of a resistant infection, it may not be a good idea, specially in view of chest infection.
I also suggest-
Steam inhalation 4 times a day
Warm saline gargles 4 times a day
Plenty of fluids.
Meeting an ENT specialist may be needed.
