Brief Answer:
ECG should be taken to rule out pain caused by cardiac ischaemia.

Detailed Answer:
Hi

Thank you for asking HCM.

I have gone through your query. ECG should be taken to rule out cardiac problems like ischaemia. There is more such risk if you have thyroid problems along with diabetes and hypertension. If that is clear then it can be most likely musculoskeletal pain. Peri arthritis of shoulder joint is also a common condition in diabetic which gives such picture. In such case over the counter NSAIDs like ibuprofen ( motrin ) , ice pack application, physiotherapy and exercises will be helpful.

Hope this may help you. Let me know if anything not clear.
Thanks.
