What Causes Pain In Lower Abdomen Post Intercourse Following Hysterectomy?

Question: I am 8 weeks post abdominal hystorectomy and after 1st intercourse since surgery I am very loathed and can't take a deal breath without pain. Any ideas what it could be?

Thanks for the query to hcm,
If there is a pain in lower abdomen post intercourse following hysterectomy then it is likely that infection has set in in the pelvic cavity if the vault had been left open during surgery and you had intercourse earlier than 6 weeks-8 weeks.
I think you should get a gynaecological review soon and get a per speculum examination and a high vaginal swab done again to rule out any infection being transferred during sex.
This is likely because healing of the vaginal cuff and sutures which have been taken on ligaments generally heal by 90 days and hence it is generally advised to avoid intercourse for atleast initial 3 months.

It's been 9 weeks and the pain/ bloating was immediate. My gyn never mentioned a vaginal cuff

Hello,
if pain was immediate then likely that the vaginal vault was not closed to allow drainage of collections( fluid or blood). And intercourse would have made it raw again.
Hence suggested that you should be visiting your gynecologist for a per speculum exam and a high vaginal swab culture. Please don't delay further.
Did you get any sort of bleeding following intercourse ?

No bleeding and pain passed after about 1 hour

Hello,
The pain during deep breathing is likely when the diaphragm is pushed all the way into the abdominal cavity and as healing would be still occurring , there would be adhesion formation internally which can form adhesions intra-abdominally , causing a stretch pain when intra-abdominal pressure is increased on deep breathing.
however if the infection is ruled out then the pain can be controlled with NSAIDS and early healing can be promoted with starting chymoral forte tabs with vitamin -C supplementation.
This if the pain is in the lower abdomen.
If there is a stretch pain in the lungs/chest region during inspiration then there is a possibility of post surgical atelectasis which can cause pleuritic pain .

Bloating can occur due to decreased peristalsis in intestines secondary to infection or adhesions leading to dyspepsia and gas formation due to increased transit time.

Nextly the instant pain after intercourse was when the vaginal vault was disturbed and then after 1 hr the vault would have returned to its original post surgical state and the stretch on surgical wounds would have reduced again leading to relief of pain.

It would be better if you can get an ultrasound as well, along with the exam.
it would be difficult to speculate without any investigation being done. So request you to review with reports.
