Question : I'm experiencing a sev
ere sharp pain in my lower right flank. seems to be radiating up and down leg. no kidney stones but have had episodes of hydronephrosis in past. starting to be difficult to move any ideas?

I read carefully your question and I understand your concern,

A pain in right lower flank can be from several cases.
--First you need to rule out the emergencies. Appendicitis is one of them which is very common. It gives pain on rebound tenderness, and also nausea, and the pain wont get better but worse.
--Other type of pain can be from a ruptured ovarian cyst, especially if you are 14 days away from the expected menstrual period.
--Yes, kidney stones can be a cause. The fact that you have had hydronephrosis makes it more possible. You should have urinary problems as well, fever maybe, costo-vertebral tenderness, ect.
--Pinched nerve pain can sometimes radiate in that pattern. Have you lifted weights recently?

If I was your caring doctor I would recommend to go in the ER so the doctors would do an ultrasound and see what is happening there, would do a CBC and see if there is infection/inflammation, drink plenty of fluids, don't take medicines if you can access the ER fast so the symptoms wound be masked.

A sharp pain, especially if is not getting better, but worse, needs to be evaluated immediately.
