Painful nipple in a male can be a sign of gynecomastia. This condition is caused by benign breast grow due to hormonal stimulation. Gynecomastia can be a sign of testis cancer, breast cancer (male breast cancer), liver problems (cirrhosis) or hormonal imbalance.

I suggest you to consult your doctor. Breast ultrasound, testis ultrasound, liver tests and abdominal ultrasound should be done at first.
