Question: I am having major pain in my right lower quadrant. I had an appendectomy approximately 7 years ago. There is a visual bulge by where the pain is. No fever, but it is hurting more to move.

From your description, hernia in the scar is the most probable reason. Please note that appendectomy surgery very often causes hernia in the scar. Its because when you remove the appendix, abdominal soft tissues are infected. And infected wound always heals worse that the clean one.

Please perform simple test. Put your hand on the scar area and push it gently. While holding the hand, try to cough couple of times (do it standing). Protruding lump or pain confirm the hernia.

I suggest you to consult general surgeon. Soft tissues ultrasound can be done to confirm the hernia. And surgery is necessary to repair it.
