Well, this kind of pain (if you are not anemic) may be due to heart problems and lung diseases, kidney function failure to some extent.
If heart doesn't pump properly, body doesn't get enough oxygen. The lungs working fine (proper ventilation is there) But there is compromised Perfusion due to less blood supply to lungs to be available to get oxygenated. This causes shortness of breath on exertion when blood demand increases and ischemic pains in the are.
Lungs--if having some disease--heart being normal--now there will be proper perfusion by heart but not enough ventilation to oxygenate the blood. The consequences being the same as were with heart disease.
Kidneys if don't properly excrete wastes and function is compromised, there is back pooling of in-excreted fluid and causes fluid in lungs accumulation. This leads to shortness of breath.
To rule this all out I need your:
Comprehensive metabolic panel (with kidney, liver thyroid and lipid profiles).
