Brief Answer:
As described

Detailed Answer:
Hi.
Muscle strains do not cause pain in testicles.
Muscles below his rear end cheeks may be due to muscular.

The causes of pain in the testicles can be due to an infection called epididymo-orchitis / varicoleces meaning collection of enlarged veins in bunches.
This can be easily diagnosed by tests of blood, urine- routine, microscopy, culture and sensitivity, ultrasonography along with color doppler examination and clinical / physical examination by a General Surgeon or Urologist ( Surgeon).

The final diagnosis can tell us whether this is serious or not / whether He needs medical or surgical treatment.
Till then ask him to take a plenty of oral fluids, have a scrotal support, avoid strenuous work.

He says even the sides of his hips are sore.
He took a hot bath and that seemed to help
