Read the history and understood your concerns.
Since there is so much of a pain, there has to be a reason.
This may be due to fecolith impaction or colitis.
Anythting related to kidney can also cause similar problems.
I think you need the following in such a situation:
Clinical evaluation and examination by a General surgeon.
X-ray of abdomen and ultrasonography.
Tests of stool, urine and blood.
Further investigations as from preliminary clinical diagnosis and reports of the investigations.
All these will help you to get a proper cause hence possible to get the proper management.
