What Causes Pain Under Sinus Region And Dialated Pupils?

Question: My fiance has the following symptoms: pain under her right eye, localized in the sinus region. Her right pupil is not the same size as her left, it is always slightly more dialated. When she was born, she reported having had a surgery to re-connect a tear duct, but her right eye still has difficulty tearing. When she does cry, she feels an intense burning sensation in the area below her right eye. She has been diagnosed with shingles, but cocurrently also suffers from migraines (per her doctor.) While some of the pain is clearly shingles related, the underlying migraine esq pain seems persistent and pervasive - but no one has been able to diagnosis her yet.

I have noted your fiancé's symptoms. It is unfortunate that a clear cut diagnosis has not been made, resulting in inappropriate delay in treatment.

Based on the symptoms, her pain is a combination of shingles pain and migraine.

For pain related to shingles, the following medicines could help- carbamazepine, amitriptyline, pregabalin, etc.

For migraine, we need to use preventive medications, such as topiramate, divalproex, or beta blockers. These medications reduce the severity and frequency of pain.

In addition, she should have a detailed eye check up to look for the status of tear duct (for which surgery was done) and an ENT check up for excluding sinus infection.
