Brief Answer:
You should get done PFT first.

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.
You should first quit smoking as soon as possible.
It is not good habit.

Since you are improving with ventorlin, possibility of bronchitis is more.
So get done PFT (pulmonary function test).

Please let me know

1. Have you ever undergone PFT?
2. Since how many days you are having this breathlessness?
You may need regular inhaled treatment on the basis of PFT.

Please reply me answers of above asked questions, so that I can guide you better. I will be happy to help you further.

Wish you good health.

Brief Answer:
Are you motivated enough to quit smoking?

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.

I can understand your concern.
You should definitely get done fresh PFT.

You might have developed some defect (obstructive) because you were asymptomatic at the time of last PFT and started symptoms since 4 months.

Another thing is better to enroll yourself in pulmonary rehabilitation center where smoking cessation is practiced.
Please let me know :

1. I can understand that it's a real tough thing to do but are you motivated enough to quit smoking?
2. Why do you smoke? I mean do you have stressful life?

Please reply me answers of above asked questions, so that I can guide you better.

I will be happy to help you further.

Wish you good health.
Thanks.

I do want to quit. I keep telling myself this is the last one, but it isn't. i did quit and was using electronic cigarettes, but I went back when I moved my mom out here and that was stressful. I was thinking if using those pills they have out. Like, I said, short of breath doesn't happen when I sit. thanks!

Brief Answer:
You should start nicotine replacement therapy (NRT).

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.

In my opinion, you should start Nicotine replacement therapy (NRT) for smoking cessation.
Psychiatrist consultation, counselling, motivation etc are also important along with NRT to quit smoking.

Don't worry, you will definitely quit smoking. Have faith in yourself..

I forgot to ask about heart issue as this can also cause breathlessness on exertion because you are active smoker and hypertensive too.

So please let me know

1. Is your blood pressure under control?
2. Which drug you are taking for hypertension?
3. What was your last 2D echo report?

Please reply me answers of above asked questions, so that I can guide you better. I will be happy to help you further.

Wish you good health.
Thanks.

Brief Answer:
Yes, this suggests that you are asymptomatic at rest.

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.

Yes, you are asymptomatic at rest and symptomatic (breathlessness) on exertion.
This suggest that your body is not good in exertion.

It does not meet the demand in exertion.
Most common causes for this are lung (bronchitis) and heart (heart failure) related diseases.
Since your ECG is just 3 months old, possibility of heart related diseases is less.
Since you are smoker and improving with ventorlin, possibility of bronchitis is more.
So get done fresh PFT.

It will not only diagnose bronchitis but will also tell you about severity of the disease.
And treatment of bronchitis is based on severity only.

You may need regular long acting inhaled bronchodilator (formoterol or salmeterol) and inhaled corticosteroids (ICS - budesonide or fluticasone).

And quit smoking as soon as possible.
Don't worry, you will be alright.. Consult pulmonologist and discuss all these.

Hope I have solved your query.

So give you best effort in it. Take help of NRT and counselling in this.
And believe me you will definitely quit smoking and be alright.
