Question: Hello. I am a 42 year male. Weigh 280 lbs. Been walking to lose weight but last week developed a small painful bump on my scrotum in the rub zone area of my thigh and scrotum. The painful bump is not deep. No worry with any STD. Shaved recently so was thinking in grown hair. Very painful to wear underwear due to location.

Superficially located bump on the scrotum is most likely a kind of skin infection. As scrotum is covered with hairs, hair follicle inflammation seems most probable.
Pain indicates that the lump is filled with pus. As long as the pus wont be evacuated, the pain wont go away and so is the lump.

If its a small lump, you may try to puncture it with sterile needle and drain the pus (if you are able to do that). If the lump is large, you should consult general surgeon and have it incised and drained.

Antibiotics might be necessary in the lump treatment. You should also keep the scrotum area clean and dry. You may wash it with water and soap.

If such lump is not for the first time, you should get tested for possible diabetes.

That is the best answer I can give not being able to see it.
