Painful ejaculation can be due many causes.They are as follows:

- BPH(benign prostatic hyperplasia)- occurs after age of 60.
- Prostatitis, an inflammatory condition of the prostate gland which can make
the prostate feel sore and irritated.
- An infection (including sexually transmitted infections) that may inflame the
area around the penis.
- Urethritis, a swelling within the urethra
- Nerve damage to the penis.
- Blockages/Constriction in the ejaculatory duct.
- Psychological problems and medications.
- Prostate cancer.
- Orchitis- inflammation of testis

From your age first possibility is some prostate related problem likely.

You need to go for PSA (prostate specific antigen ) testing as well as ultrasound examination.

Please answer some questions for further help.

- Do you have any problem in urinating process?

- Do you have any burning or discharge while urination?

- Do you or your wife have any history of STD(sexually transmitted disease)

- Is there any history of back pain or injury over pelvis/back region?

- Do you have any pain in testis or injury over there?

in response your questions 1-4: No
injury from kick in groin perineal area by madman 15 years ago and subsequent inguinal hernia surgery some 5 years back gives occasional pain some time after intercourse, but this does not intuitively feel like source of ejaculation pain which is immediate and very intense and overshadows former feelings of a "happy ending"

Hello,

Thanks for follow up. Ejaculation involves seminal vesicles of prostate and urethra of penis. Timing of pain during sex suggests something wrong in either of these. Please consult surgeon for physical examination and revert back to me with investigations suggested in last answer.

Treatment depends on cause.

Please do not delay. Prostate problem if diagnosed early, better the outcome.
