I am Dr. Kakkar. I have gone through your query and I have also looked at the picture.

I can see in the picture that there is a ring shaped/annular lesion which shows a central clear area. The peripheral part/margin of the lesion is red, raised and seems a little scaly at some places (12 o'clock position).

This type of presentation can be seen in cases of Fungal Infection. I guess it must be itchy too? even though you mention it to be painful. Is pain the result of excessive scratching?

If I was the treating doctor I would have treated it only after confirming the diagnosis by scarping a small amount of scale from the periphery of the lesion and see it under the microscope for any fungal elements (Hyphae).

If the lab test reveal the presence of fungal elements, you may try an OTC antifungal cream available from a nearby drug store, till you are able to see a dermatologist. Topical antifungals alone may be effective. However, since this seems to be an extensive lesion, therefore, if I was the treating doctor I would have also suggested oral antifungals in addition to the topical cream. Oral antifungals are prescription drugs and therefore it is mandatory to get a prescription from a doctor.
