Question : Hi I have a very painful swelling between my anus leading up to the base of my scrotum... It started about 4 days ago but now the pain is extreme.

Thanks for your query, albeit short and specific.
To recapitulate: Male/45 - very painful selling between anus leading up to base of scrotum - 4 days - now the pain is extreme - taking Advil and hot compresses ....

This is indicative that there is an abscess formation as this is 4 days old with extreme pain.
I would advise in such situation the following:
Go to ER as you need a clinical evaluation, examination and incision and drainage as early as possible other wise there can be toxemia; needs proper antibiotic cover and Advil in proper dosage and duration.
