What Causes Painful Swollen Yellowish Scabs Around Anus?

Question : My anus burns and hurts very bad, you can see little yellowish/brownish scabs around it. And the middle red area is swollen. I'm negative for hsv2 (swab and blood test was done) and was just diagnosed positive for strep in my genital region. This happens for a day or two days then goes away. I was on valtrex and that didnt help. I'm currently on cipro and still got this. I'm also applying colloidal silver, hydrocortisone, antifungal, antibacterial on it. I'm on my second day of cipro. What could this be? I'm scared the cipro isnt going to work and its not being treated correctly. Also, prednisone helps with swelling and discomfort but i cant get that all the time.

Hello and Welcome

I appreciate your concern.

I have read your history carefully. The topical medications which you are using are inappropriate and in excess and need to be stopped.

Ideally you should continue the Ciprofloxacin but i would advise adding Metronidazole ( Flagyl) to this regimen which will cover anaerobes. Your physician will know about the dose.

Topically you may use GTN ( Glyceryltrinitrate ) and a combination of Bacterial Culture Suspension and Hydrocortisone eg Posterisan forte. If pain is severe you may also be advised Topical lidocaine or lignocaine which are local anaesthetics.

You should continue the above regimen for at least two weeks in consult with your primary care physician.

If symptoms persist, I would advise you to consult a General Surgeon. DRE ( Digital Rectal Exam) should be done to rule out hemorrhoids and anal fissure should also be looked for in the initial assessment.

Does hsv1 have any of these symptoms or apperence? I dont feel like i get anything prior to getting these scabs, i just start feeling irritation and these scabsare here. Ive had about 5 of these 'outbreaks' in the past 4 weeks.

HSV 1 is more commonly associated with oral and facial lesions though just based upon the lesion diagnosis cant be established. Both HSV 1 and 2 can present with similar lesions.

Tzank test can be performed if you are worried about HSV, though it cant differ between HSV 1 and 2. Tissue culture isolation of the virus is the best test available for confirmation of HSV.

You should complete the antibiotics course, and then re evaluate at the follow up visit whether a surgical consult is required or not.
