Question : Yes ty, I feel like my bladder is fallen , It hurts when I pee and I find myself holding up the bottom of my stomach when I walk. I have had a bad cough last 2 weeks and have strained to go to the bathroom. I have been very constipated. I already had a complete hysterectomy and I am 67 years old. I also had a umbilical hernia and they fix it with mesh back in 1993. Can you tell me what it sounds like to you and what will a doctor do, and it be fixed?

Brief Answer: as described in details down Detailed Answer: HI. Thanks for an elucidate history. Noted your Medical History which says about previous surgeries and high blood pressure (which is normal). Since When have got this problem of constipation and hurting at pee? Is it since the previous surgeries or just recently? If this is recent , it can be related to the cough and the constipation. The straining at both could have caused the problem with the sphincter( which is single in females). That in turn can cause hurting at pee. The pain causes incomplete evacuation of the bladder and gives the feeling of fullness and falling down. In addition the constipation / impacted feces also pressed on the bladder-neck casing the similar problem. Taking a laxative and/or enema will remove this cause. Taking good anti-tussive (cough syrup) will help in no straining. Please see that it does not contain codeine as it causes constipation. Take time to pass the urine try to relax- Try to avoid straining so that the edema does not increase. If this is of a long standing and has started after the surgeries , the cause is obviously cystocele meaning the bladder has fallen down per-vagina. This needs to be checked by the Gynecologist on real inspection under vision particularly on straining. If this is present , you are required to undergo the Corrective surgery- repair of the cystocele.

Yes it has just started a cuple of weeks ago. What is a sphincter? I hope it is something not serious. Should I go to an emergency room or just wait until I get in. I am taking tramadol which is the only thing I can take with a stomach that is very acidity. They found that out last year when the gas was going up or it felt like it and I thought I was having a heart attack. I forget to mention I also had 2 gastroplaties in 1987 and 1990. It is very painful now and I can take alot of pain without medicines. Thank you for the opportunity to talk to you. XXXXXXX

Brief Answer: not serious Detailed Answer: Hi, Good to know this is just a few weeks old. Your problems may be over just by following as advised. Sphincter is a muscular ring which opens only when you have to empty that particular organ. For example in bladder it is situated at the bladder-neck area- just where the bladder terminates. Our body has many- They allow the forward flow on opening , otherwise always in a closed position and also does not allow retrograde flow (backwards). I do not think it is something serious. You may just wait.. Tramadol in an adjustable doses keeps you pain-free and is quite safe. Regards,
