Brief Answer:
Most likely a urethral injury

Detailed Answer:
Hello,
I can understand the stress you must be through,
As I can understand from the information given, there was a passage of clot from penis. Although here it need clarification whether it was a clot or fresh blood in stream of urine which is more common symptom in men.
I would need to know from you, if there was a trauma to his penis or lower abdomen or to pelvis in past 24 hours.
If this there is any pain or burning sensation associated while micturating?
Is this his first episode, or was there a similar complaint in past?

Here my possibilities after reading your given history:
Urethral injury is the most likely condition that presents with such complaint.
Any physical trauma over penis, fall over abdomen can cause urethral injury.
Prostate problem causes problems in passage of urine, such as incomplete emptying of bladder, etc, but not blood in urine.

As there is high possibility of urethral injury, I suggest you immediately show to your physician for clinical examination if possible urologist. Exploration of injury will be done, depending on which catheterization can be indicated.
Do not worry as complete recovery can be possible without any functional loss.

Welcome for the further queries.
