I have gone through your query and I have also reviewed the Images. See seems to have hand eczema Or chronic cummulative irritant contact dermatitis.
I suggest her to use an intensive moisturiser e.g Amylac cream mixed with an equal proportion of a potent topical steroid i.e clobetasol propionate 0.05% cream e.g tenovate cream (1:1), twice daily for 2 weeks and thereafter just continue with the moisturiser. Avoid repeated washing with soaps.

She is already taking it...no kind of improvement seen.
