Question: Hello, I live in a country in East XXXXXXX where Malaria and typhoid is common. On monday I started having a feeling of being sick. I ended up having a high temperature during the evening time and went to the hospital where I was given an injection of Paracetomol. My temperature went from about 38.5 to 37.5 degrees celsius and they sent me home, but I still didn't feel all that great when leaving. Later that night when going to sleep my temperature went back up a lot I think to like 39.5 and I felt terrible headache, shivering, no appetite, and felt like vomiting at one point (i didn't). I went back to the hospital and this time they did bloodwork and told me that I had typhoid. They gave me an injection of Ciprofloxacin which dropped my temperature a lot and I felt better. They then gave me ibuprophen, paracetomol, and the Ciprofloxacin all in tablet form for me to take. On Tuesday I took the paracetomol and ibuprophen every 6 hours as instructed and also took the antibiotic after 12 hours as instructed. I felt pretty good for most of that day (Tuesday) but when I went to sleep at night my temperature started flaring up badly all the way to 39.5 degrees celsius again with same symptoms from the first night. I went to a different hospital this time and the doctors there did more blood work and also said I have typhoid based on the blood work. They gave me a paracetomol injection that brought my temperature down to 37.5 but then it went back to 38.2 within an hour. Later on around 9am a different doctor reported to work and said he didn't think it was typhoid and instead thought it was a viral infection. I have given them a stool sample to test but no results yet. They said I had to be admitted to the hospital because of my symptoms and persistent fever. My fever got back up to 39 and they gave me more paracetomol after 6 hours and the temperature went to 37.9 for now and then back to 38.1. All tests thus far have said it's not malaria.

Yes the test for typhoid is positive.
But your blood picture and urine examination are normal.Sometimes this kind of uncontrolled fever happens. But since blood counts are fine, I would not worry.
I think you should continue with Ciprofloxacin and I would like to add Doxycycline.
Regarding paracetamol and ibuprofen--take only crocin /paracetamol-4 hourly.
Maybe you need Omeprazole to prevent acid build up, and a B complex.
Add alcohol/spirit/aftershave/spray scent to a small bowl of tap water and sponge with this whenever temperature rises.
Make sure you drink a lot of water and fluids and eat a bland nutritious diet.
Carrot soup is excellent for typhoid.
Discuss with your doctor about Doxycycline, or even Chloramphenicol.
Avoid injections.
Blood counts should be repeated every weak.
