Question : Hi, for over three months now I have been experiencing headaches. Doesn't really hurt, just feels like a pounding on top or at the fore front of my head. It disturbs my concentration. Sometimes feel like something moving around, but tend to feel slightly better when I wear a cap or press something on my head. It's a accompanied lately by sharp tingling sensation in toes, fingers, and other parts of my body. I experience sore joints and muscle pains. Went for a run yesterday and stop running because of pain in my calf.

My full blood work was done twice and tested positive for lymph and I had high cholesterol. Was treated for lymph, but has not experience much change. MRI of the head and neck also came back ok. It said "there are multiple punctate areas of FLAIR hyperintensity involving the white matter both frontal lopes particularly in high convexity levels". Doctor says there's nothing alarming, but still feel quite unwell. Please help me. Thanks

Brief Answer:
MRI Brain favours a diagnosis of migraine.

Detailed Answer:
Hi,

Thank you for posting your query.

I have noted your symptoms and MRI Brain reports. First of all, I would like to reassure you that there is no need to worry about the report, as it does not suggest a diagnosis of any serious brain disease.

The most likely diagnosis in your case could be migraine, as these type of MRI brain findings are common in migraine (vascular headache).

Though similar MRI findings may also occur in ischemia (lack of blood supply to brain) and multiple sclerosis, these are unlikely in your case.

Since you have frequent headaches, you should be started on preventive medications to reduce the severity and frequency of headaches.

You should also do blood tests for vitamin B12 and vitamin D levels to exclude their deficiencies.

I hope it helps.

Best wishes,

Brief Answer:
Vitamin deficiency needs to be excluded.

Detailed Answer:
Thank you for getting back.

I noted all your symptoms and these symptoms can not be explained on the basis of MRI findings.

As mentioned, we need to check levels of vitamin B12 and D in the blood as well as thyroid profile, as their deficiency may cause weakness, tiredness and tingling.

Best wishes,
Dr Sudhir Kumar MD DM (Neurology)
