Question: Hello sir, Past Health Info: I had a Postherptic Nerulogiaa on left flank. Which went off by taking medicine primox. Age: 26 years (Male) Present Symptom’s: 1.on right side flank or torso I am experiencing pain and its more than 3 months. 2. Also pain in right leg all over including my feet. It's limited to the right side only. It's not constant, but it comes and goes. 3. Tingling sensation on legs while applying pressure. 4. I feel tremors while sleeping. Note: Kindly note that PHN was on left flank, but now it’s right flank. Tests: Ultrasound done last month was normal. Blood test and Urine test done before 6 months was normal in range. Medicine: Currently on Pregabid-NT for 2 weeks but no improvement with my right side leg pain. I am worried about right leg pain. Thank you in Advance. xxxxxxxxxxxxx

I have very carefully gone through the details provided. Although you had PHN, it is likely to remain limited to the respective region of nerve supply. Since a bilateral involvement is there and since radiation to lower limbs is present, I strongly suspect that a component of compression of spinal nerves is involved. It is possible that the lower back muscles are going into spasm and thereby resulting in the pain that you are experiencing. If that is the case, hot compress should provide relief. However, you should visit a physical therapist who would be able to demonstrate the right exercises that you need to practice on daily basis. In more severe cases traction might be required.

If you have the condition I mentioned, response to the existing medications is likely to be limited. Ultrasound would not be relevant in such a scenario or would not yield valuable clues.

Thank you Dr. for your response.I developed PHN on my left Flank before 1.5 year, but now its right flank and right leg.I am bit curious how it links each other.The leg pain is only in the right side which is from my hip to feet and liitle devastating also.Shall I continue with existing medicine? Any suggestion from your side how to get out of it.Or else its some other disease which needs more tests and medication?Thank you in advance.

Thanks for writing back. I would insist you to continue the existing medication until your treating physician insists not to. About relief from the pain on right side, try hot compress and get in touch with a physical therapist.
Getting an X-ray of lumber spine - AP and lateral (needs bowel preparation first) should be helpful.
