Question: I am using Propysaic NF 6 since last 5 years as a treatment for psoriasis ... The condition s better but not cured , What can i do now

I am Dr. Kakkar (Dermatologist). I have gone through your query and I have noted uour concern.
Propysalic NF 6 ointment is clobetasol propionate and salicylic acid combination. It is a potent topical stetoid preparation. Though topical steroids are one of the treatment options for psoriasis but they should not be used continuously for longer than 4 weeks because they can not only cause side effects like skin atrophy but topical steroids if used continuously for more than 2-4 weeks become ineffective because of a phenonmena called tachyphylaxis. Therefore potent topical steroids should either be used for a shorter period like 2-4 weeks or there should be a steroid free holiday peroid to maintain efficacy.
Other options like topical vitamin D analogues like calcitriol, calcipotriol are non steroidal and safer alternatives. These can be either used as altenatives to topical steroids or during steroid free holiday period.
Apart from these treatment options for psoriasis also depend on the extent of skin involvement. Widespread psoriasis generally does not respond to just topical treatment and so these patients may require systemic treatment like methotrexate, cyclosporin, phototherapy etc.
I would like to know what is the extent/ area of psoriatic skin involvement in your case? Is it localized or widespread patches?
