Question: Just to follow up and clarify... Because the botox healed the anal fissure about 3-4 months ago. How long does it take the tissue to become strong again. and since the toxin binds to presynaptic nerve terminals? Why do the muscles around the rectum/tailbone area are constantly sore and achy like a dull ache higher up? Not sure I understand, so when the botox wears off that sore dull muscular ache will wear off gradually or or go away all of a sudden after 6 months or so?

I have gone very carefully through your query. The resolution of anal fissure provided by botox is temporary and not permanent. It will not make the muscles stronger and would not help in resolution of the pain that you describe. It is not unlikely for the problem to resurface from time to time.

Fissure is frequently associated with hemorrhoids which in turn is most often associated with constipation. To control it you need to get rid of constipation. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. Green leafy vegetables and other sources of dietary fibers contribute roughage which for the bulk of stool. Unless fiber intake is adequate, the gastrointestinal movement will be hampered and piles will result.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Avoid red meat.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fiber intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
* Take Sitz bath. In a big tumbler take water as hot as your skin can take. Sit on it with your anal opening dipped.
* Taking Daflon tablet 500mg every 4 hours for 3 days followed by once every 8 hours for next 4 days should help in resolution of the problem.

I do not have constipation, I only got a fissure this one time due to a bout of Diarhea, which now that is fine, stomach is fine. It seems that the fissure had caused some muscle spasms with the surrounding muscles, levator, the sphincter seems to have relaxed. Just feels like the connective tissue needs time to settle down as the previous doctor explained I just didn't quite understand, the whole answer. There are two internal hems and have been taking diosimin supplement which helps, there is no bleeding and no other symptoms other than the tightening feeling higher up in the muscles near the tailbone. I read it could take 6 months for this to get better and go away completely.

Thanks for the update. Diarrhea too causes stress on the anal sphincter and therefore could be contributory. Diosmin is a flavone similar to Daflon and therefore should help.

It could sometimes go away in 6 months but unfortunately that is not always the case.
