What Causes Persistent Swelling In The Ankles?

Question: I have an ankle that is swollen ,red ,hot and very painful. 4 days now . had ultra-sound no blood clot .

Please give additional information for me to assist you better:
Please post clear and focused pictures as it helps sometimes for on-the-spot diagnosis.
Was there any history of twisting injury?
Any direct injury that cause breach in skin?
Fever, lymph nodes in inguinal region on the same side?

Red, hot and painful ankle is indicative of an infection in or around the joint.
A course of an antibiotic may be necessary along with an anti-inflammatory medications like Ibuprofen if it suits you.
Further investigations of blood and MRI if possible can give more perfect diagnosis and give proper plan of management. Also needed may be blood tests of RA factor ESR etc as per your Doctor's advice.
Till then give rest to the ankle.
Elevate the affected area above heart level in laying down position to reduce edema.
