Question : My hands and feet feel like they are on fire/pins and needles when I wake up in the morning. It is severe enough to limit my ability to use them without pain....like typing this! My physical isn't till february and I want to know if I should be seen before then.

I read your question and i understand your concern. These pins and needle sensations are paresthesias and considering your history fibromyalgia, diabetes history and medications amitriptyline in specific you take, are all responsible for these numbness and pin and needle sensations. Also diabetes predisposition are notorious for these pins and needles sensations even at its earliest stage called peripheral neuropathy.

With that much illnesses diet and nutritional neuropathies also kick in and B12 deficiencies along with other vitamins supplements also get responsible for these reasons.

Morning stiffness and more sensations in the morning may also consider a rheumatological profile like rheumatoid or any autoimmune rheumatological condition like lupus may also needs to be put on the table.

Nut shell, this paresthesia needs to be correlated clinically and some deep through labs need to be run to sort out the cause and reach to the base of it.

As you already have a physical in february , i am sure it can wait till then. Its not an emergency. Just try to be on precautionary measures till then like fibromyalgia prophylaxis of diet and lifestyle and minimizing the doses of amitriptyline to the minimum. Keep exercising and try to eat healthy and vitamin rich foods and take some B12 rich foods especially fish and liver and meat as these are rich in B 12 .

Once the february reach which is a couple of weeks now , then get evaluated for the etiology underneath. I am sure it will be a good news.
