Question: Hello recently had a pcr test and come back positive for ureaplasma and gardenerella what is this what do i do now ? Thanks

Good afternoon.

As it seems you've already discovered from asking other doctors on this network the microbe ureaplasma detected in your case after having had unprotected sex from a new partner is the most likely way you were transmitted this organism. You may be suffering from a very mild case of male urethritis though pain doesn't seem to be part of your picture. It's not uncommon to find ureaplasma in culture specimens from patients without any real symptoms.

In your case, I would ask that the organism be cultured and antibiotic sensitivity be carried out because treating blinding is about 50/50 accurate. Most common medication of choice would be doxycycline for 7 days with clindamycin being in 2nd place. Other antibiotics are possible but usually not necessary beyond those two.

You then, need to get your partner tested and treated.

As far as the Gardnerella species that was also cultured...well, traditionally, it was always thought the this bug didn't cause too much problem in the male though it is well known to be the cause of the foul fishy odor and grey discharge found in vaginal secretions in the females. It is clearly transmitted through copulation in the male though in the female it can occur as an infection by other means other than sexual means.

Whatever the case if you are having any penile discharge, if there is any odor to your seminal secretions that's fishy in nature then, the gardnerella bug is probably actively participating in your clinical picture.

If not, then, it may not be causing any clinical problems. I would check with your partner to see if she is having any symptoms of a bacterial vaginosis (smelly secretions, etc.) in which case she definitely needs treatment and then, so do you...but she is clear of any symptoms of anything, then, you may not need treatment for the Gardnerella part of the equation.

Having penis like mucous dischage after i wee could the ureaplasma been there for many years and now just turned up ? I am really grateful for your time and help .they did not do a culture it was a pcr by post and now will go to my gp with the diagnoses and see what they say

Thank you for your return question. If the discharge has a foul odor to it then, there is DEFINITELY something infectious that should be treated.

If there is no odor and it is a minimal amount then, it could also be seminal fluid (easily identified through analysis by sending to the lab).

If the discharge has been there FOR YEARS and/or before your last partner that you say you recently had unprotected sex with then, yes you may be having a chronically colonized infectious process. Again, it needs to be send for culture and sensitivity and treatment offered depending upon the results.

Once again I would underscore the IMPORTANCE of your

1. Finding out what organism your urethral discharge and/or urine is harboring and get treated. But more importantly you must engage ONLY in protected sex going forward unless you KNOW exactly the background and infectious status of your partners.

2. You should not engage in any sexual activities until you've been fully treated yourself since you are currently contagious and even with protected sex you can still easily transmit your organisms.

3. You should collect a list of sexual contacts that you've had ever since noticing the urethral discharge or the urgency to go to the bathroom and alert them to the need to get immediately checked for STD's whether or not they may be symptomatic and to also recommend they not engage in sexual contacts until they are cleared for infection or treated.

In this way you can help break the chain of transmission of these infectious entities which can cause a lot of harm to both fertility of a partner and other complications.
