What Causes Pounding Heart Beats Everyday While Having Rest?

Question: Hello, i am male and 32 years old.

For the past 8 months I keep experiencing very hard heartbeats everyday at rest. It feels like my body rocks with every beat.

I experience no pain but get tachycardia with minimal exertion and my pulse rate when standing is usually in the high 90s.

The pulsations are visible in my stomach.

The palpitations normally tend to start in the afternoons and I continue to feel them when I am asleep.

Sometimes when I fall asleep just before I lose consciousness I feel as though my chest starts to tremor, maybe it's adrenaline enducing this im not sure. When I wake myself up the tremor slowly subsides within a minute. I do not think this is an arrhythmia as a 24 hour Holter monitor detected nothing abnormal when this occourred.

A resting Ecg shows IRBBB.

I had an Echocardiogram 6 months ago and it showed nothing abnormal.

Blood test for thyroid is normal.

24 hour urine collection to check horemones normal.

Blood pressure found to be high because of right kidney abnormality. Right kidney is atrophic with scarring and working at 30%.
Nephrologist believes this was caused by a urine infection.
Blood tests show normal electrolyte levels and renal function appears normal.

Blood pressure controlled by 10mg Amlodipine, avarage reading 140/80.


What do you believe might be the problem here?

I read your query and understood your concerns.

First of all my advice is to do all the needed examinations to find out the exact cause of your problems. You are reporting that you have done already cardiac echo, hormonal analysis and abdominal ultrasound. I presume that you haven't done any other examinations. However if you have done any other test could you please send the result to me?

My opinion is that what you are experiencing is more related to:
1. arrhythmia or
2. some anomalies of aorta and renal arteries that may be the cause of right kidney atrophy.

This is why i advise to run the following examinations to find out the exact cause:
- Holter monitoring (to exclude arrhythmia)
- Repeat the Cardiac ECHO (as a rule it should be done every six months)
- Chest x ray
- CT Angiography of aorta.

The other test I have had is an MRI to check heart and kidneys. The results showed that the heart is normal in size and that there is no stenosis in the kidneys.

A few more symptoms I forgot to mention:

As well a visible pulse in my stomach, I sometimes notice a visible pulse in my left arm.

I have noticed that my heart rate is also sensitive to stress, anxiety and excitement.

My posture can also contribute to tachycardia, for example ...bending down or prolonged standing still.

Is there any chance that my medication (Amlodipine 10mg) maybe causing or contributing to my symptoms?

Would the MRI have shown any abnormality with the Aortic or Mitral valve or is that something that only an Echo can confirm?

You mention the possibility of an arrhythmia. Bearing in mind that a resting ecg has found IBBB, Is there any specific arrhythmia that can cause a bounding pulse at rest?.

1. The visible pulse on your stomach most probably is related to abdominal aorta being too near to the abdominal wall.

2. If something wrong (e.g. aorta aneurism), the MRI you did could have shown it. If no such report in MRI results, then, no aneurism.

3. To answer specifically to your queries, I need to have the full report of the MRI examination you did.

4. Having tachycardia which is felt as heart beats is a normal reaction to stress, anxiety and excitement. Holter monitoring is advised to rule out arrhythmias (such as cardiac extra beats) which also may be the cause of heart beats.

Ho thank you for your reply. I will relay your advise to my GP.

I have another question.

My resting ECG results shows an IRBBB. Could a IRBBB be felt as a hard heartbeat, bounding pulse, or a tremor?.....or could a hard heartbeat or tachycardia be the cause of the IRBBB?

It's only an ECG finding, don't worry about it...

IRBBBsis only an ECG finding and can't be felt or can't be the cause of any symproms or arrhythmias.

In most of the cases it's normal without any caise, however it's not caused by heart beats or tachycardia.

You shouldn't worry about it.
