Brief Answer:
Complete evaluation and proper guided treatment needed

Detailed Answer:
Hi.
Thanks for your query and the history you have provided, albeit small.

I would like to ask a few things here:
Was CT scan or MRI done to rule out spinal cancer?
Was the lump evaluated by the same investigations?
Do you have any reports of blood / urine / stool examination?

The problem of difficulty in urinating due to pressure or restriction and other symptoms can be due to the following reasons in a female patient of 42:

There can be a mass of ovary or uterus or other organ causing pressure on the urethra/ bladder.
( Your history of Doctor finding a lump inside her in lower left abdomen / Associated symptoms of numbness in the anus and feet and swelling on her buttock )

Intermittent fever can be due to infection of the urinary tract developed secondary due to obstruction or due to the original cause causing other symptoms.

I would advise you the following:

To have a radiological evaluation by way of Ct Scan of the whole abdomen and pelvis. This will give a detailed idea about the lump causing the symptoms.
To have FNAC / Biopsy to have a pathological diagnosis.

Once a suspected diagnosis has been made, to consult the Specialist in that particular field for further treatment.
Relevant blood, urine and stool tests.

It is always better to try to get a clear picture and to accept the reality to try to get the best possible treatment.

I hope this answer helps you to get a proper direction for early diagnosis and treatment.

Wishing you all the best.
