Question: My 37 year old daughter is experiencing severe symptoms for the past 4 days. She has a tightness in her chest which is preventing her from swallowing. It is in the area of her hiatal hernia. She also has canker sores and a serious yeast infection. We have been to the E.R. twice and was given prednizone and an antibiotic. We have made an appointment with an ENT specialist, but that is Wednesday. She is in pain and cannot eat anything. I honestly do not know what to do now because she needs relief.

Thanks for posting the query

Your daughter seems to be having heart burn secondary to the hiatus hernia and acid regurgitation into oesophagus and the throat. This is the prime reason for the chest and throat discomfort.

Yeast infection does occur due to stress, reduced immunity, certain medications, etc. Prednisone is not an appropriate choice at this juncture. She would require systemic proton pump inhibitors to reduce the heartburn quickly and the effects of hiatus hernia. Potent oral antacids will reduce the pain in the neck as well as chest discomfort.

Can you explain the sores in her mouth and throat preventing her from swallowing anything beyond ensure? Also, what should we do to ease this pain; she is in terrible discomfort.

Canker sore/residual oral thrush...

Hi

Thanks for writing back

Sores in the mouth could either be canker sore or residual oral thrush (yeast infection). If you can send me a picture of the oral cavity, I can help you in a better way.

If it is canker sores, then gargles with oral steroid solution can reduce the inflammation. If the sores are secondary to the oral thrush, then, suspensions like Posaconazole has to be used.

Meanwhile, the medicines for hiatus hernia mentioned in my earlier response has to be continued.

Hope this clarifies your doubts. Kindly send me few photos of the oral cavity, if possible.
