Question : I had a heart dissection in May 2014 and take heart meds. During the last 10 days I have been experiencing a tightness or pressure in my chest. I have been treating it for indigestion or reflux as I have a history of that but more times lately it does not work. I also have a prickly feeling in my middle back area which goes away and comes back. I have been using my nitro spray very recently which I really haven't used since XXXXXXX 2014. It seems to help but the same feeling returns. I don't know what to do as I am a visitor to the US and don't know where to go.

I feel that a cardiac evaluation is in order for you my friend.
I cannot be sure at this juncture whether the tightness is related to hear dissection.
But a possible new cardiac lesion in the vessels of the heart is a definitive possibility.
And yes, symptoms are similar to gastritis/GERD.
But with antacids not helping and nitroglycerine giving partial relief, I definitely think you need to get evaluated as soon as possible.
I am sure you will have a hospital in the city in USA where you are located.
Might as well meet a cardiologist for a stress test.
