Posted on Tue, 24 Mar 2015
Question: Dear Doctor,
For about 2 weeks now I have noticed I can grab a kind of chord at the back of each testicle that I believe is the Vas Deferens. What scares me is when I masturbate and am close to ejaculation it get really hard like there is a lot of pressure built up behind the left testicle is where it really gets hard but I can feel less of it in the right. I am worried it might be a blockage or something in the Vas Deferens? Please get back to me with what you think might be the reason for this. Thanks

Brief Answer:
Physical examination / Semen analysis / ultrasonography.

Detailed Answer:
Hi.
Thanks for your query.
Read and understood your problems.

To recapitulate once : young male / 19 - 2 weeks - can grab a kind of cord at the back of each testicle - Vas Deference - gets hard like there is a pressure built up - more on left than the right side - when close to ejaculation during masturbation - worried about the block - wants to know the reason...

I would say, this is really a very great observation on your part and the understanding you have.

The part which you are feeling is most probable a part of the Spermatic cord / Epididymis as per your history. If it is getting the fullness as you explained we have to search for the reasons.

The best way to get this confirmed is by the following way:
-Get a clinical evaluation and physical examination done by a General Surgeon or Urologist.
-Ultrasonography of the scrotum and abdomen with color doppler mode also.
-Semen analysis to see if the sperms count / morphology / anatomy are normal or not (will get reduced in block on one side and will be zero if the block is on both the sides.

Do not think too much till you get the results.
As the facts only can tell us / you the real problem.

Once the diagnosis is made whether this is just an inflammation causing all this or a real block or whatever, the treatment can be well planned.

I hope this answer helps you, please feel to ask for further relevant questions or if you feel that there is a gap of communication.
I am waiting for the feedback of all the tests I have asked for.
