default
Posted on Fri, 17 Jun 2016
Question: I have a lot of pin pricks and twiggling all over the body. Especially in and around my heart which made me very scared. I did and ECG, Threadmill test and all was fine. I did a master health check up and the cholestrol level was 210 and thyroid slightly higher. I was put on medication throx 25mg and fencover 105 and now both the levels are fine. however my prickling and twiggling sensation has not stopped.Any advice as my doctors say it is a muscle pain and stress.

Brief Answer:
get yourself tested for Vitamin B12 and Folic Acid first.

Detailed Answer:
Hello,
Welcome to HCM,

I have read your query and reviewed the reports you attached.

The symptoms of pricking and tingling all over the body could be related to multiple factors which should be ruled out.

The most common cause for these symptoms are,

1) Thyroid dysfunction
2) Vitamin B12 and folic acid deficiency
3) Peripheral Neuropathy.
4) Anxiety or stress related,
5) Urticaria (a kind of skin allergy) should also be ruled out.

Since you have already corrected your hypothyroidism by taking thyronorm and if your symptoms still persist,I would suggest you to get yourself tested for Vitamin B12 and Folic Acid.

In case, If above tests are normal then approach a neurologist for a nerve conduction study to rule out the chances of neuropathy though slightly unlikely here.

Tab Pregabalin should be given once at night with over the counter antihistamines (cetrizine) which would give you some symptomatic relief.

Anti-anxiety drugs (recita forte) would also be helpful to relieve from excess anxiety or stress.

I treat my patients with a one daily dose of Vitamin B complex as well.

However, Pregabalin and anti-anxiety drugs are prescription drugs and you should see your doctor for the written prescription.

Hope this helps.

Brief Answer:
pregabalin 75 mg and methycobalamin 750 mcg combination.

Detailed Answer:
Hello,
Thanks for sharing reports.

I have reviewed them.

1) TSH values is slightly high and I will suggest you to continue with thyronorm. And preferably thyroid tablet (thyronorm) should be taken on empty stomach to get maximum absorption as food might interfere with its absorption.

2) Your HDL is slightly low and LDL is slightly elevated (with Chol/HDL ratio) while other cholesterols are completely normal.

To reduce your Chol/HDL ratio and to increase HDL,you should follow these dietary guidelines:

- Avoid direct sugars and fried snacks, bakery items.
- Have ample of vegetable salads- at least 1 bowlful cut raw vegetables like cucumber, tomato, carrot and vegetables which you can eat raw.
- Snacks should contain healthy nuts like almonds or walnuts and a few dry fruits.
- Avoid soft drinks or hard drinks and smoking cessation if you do so.
- Simple Chapati and steamed rice or rotis made up of different whole grains should be the staple diet.
- Egg white is fine but avoid egg yellow means yolk.
-vitamin B3 (niacin) increases HDL cholesterol.

3) Folic acid is very much within normal limits and Vitamin B12 (432) is also normal but clinically it should be beyond 500 to 600 to avoid any symptoms.

So,it would be advisable to start with pregabalin 75 mg and methycobalamin 750 mcg combination (brand name: Pregalex-M 75/750) once daily.

Your family physician may prescribe it along with symptomatic over the counter antihistamines as we discussed earlier.

Anti-anxiety drugs (like recita forte) would also be choice and prescribed to relieve from excess anxiety or stress.

You may wait for 4 to 5 weeks to see improvement with this treatment if the pin pricks and tingling sensations are not so troublesome to you.

However,if still problem persists, then it is better to seek advise from neurologist.
