Brief Answer:
Can be Hernia with complications or Fournier's

Detailed Answer:
Hi.

Thanks for your query.
Read the history, albeit small.

I would request you to give some further details to help me out to suggest you the probable diagnosis. Please provide more information...

Since when have you got this problem?
Inflammation of the pubic area and the 1/2 of ball sac is redness, pain and tenderness OR just a swelling without any other symptoms?
All these problems occur only at night and settles in the day?
Throwing up (vomiting), can't shit ( constipation) also occurs in the night or at any time? You have mentioned this happens after you eat.

I would like to have say that from the history you have given this can be due to few possibilities.
- Constipation and throwing increase the intra-abdominal pressure to cause the hernia to pop out causing the swelling in the pubic area and the 1/2 of ball sac.
This an be an inguinal hernia.
- If here is real inflammation with pain, redness and tenderness with constipation and vomiting, this can be an obstructed / strangulated hernia; needs an urgent medical attention and may be a surgery.
- Another possibility is that of Fournier's gangrene, meaning the inflammation of the scrotal skin spread of the infection to the pubic area, the septicemia causing the other symptoms.

- I would advise you to go to ER to get a clearer picture and diagnosis and further management ASAP.

Your feedback will help me to give you more details. Awaiting for it. You may please attach a clear photograph / picture attached to this query.

I hope this answer helps you, please feel free to communicate more and get to the root cause.
